A serving size is the amount the label's numbers are based on. A portion is what you actually eat. They are frequently different, often by a large factor, and confusing them is the single most distorting mistake in food tracking.
What a serving size actually is
Under current US labelling rules, serving sizes reflect how much people typically consume at one time rather than how much anyone should eat. The FDA's page on serving size states the distinction directly.
The purpose is comparability. If every manufacturer chose its own basis, two similar products could not be compared, and a company could make its numbers look better simply by declaring a smaller serving.

The line people skip
Immediately above the serving size on a label is servings per container, and it is the most consequential line on the panel.
If a package contains three servings and you eat the whole thing, every figure on the label applies three times. Calories, protein, carbohydrate, sodium, sugars — all of it.
This is where "I only had one packet" quietly becomes a substantial fraction of a day. It is not a labelling trick; the information is printed. It is simply the line that gets skipped.
Why this error is worse than other errors
Most tracking errors are additive: you forgot to log a snack, or a database entry is somewhat off. They add a modest amount to a day.
A serving-size error is multiplicative. Getting a serving wrong by a factor of two doubles every number for that food. Two or three such errors distort a whole day well beyond anything a database inaccuracy would.
This is why, if you are going to be careful about one thing in tracking, this is the thing.
The literature on self-reported intake finds substantial inaccuracy generally — the Frontiers in Nutrition paper on the plausibility of self-reported energy intake discusses how it is assessed — and portion estimation is a major contributor.
Converting a serving to your portion
Three approaches, in decreasing order of accuracy and increasing order of convenience.
1. Weigh it. A kitchen scale removes the estimation entirely. Weigh, divide by the serving weight, multiply the label figures. It is the accurate option and few people sustain it forever.
2. Calibrate, then estimate. Weigh for a week. You will discover that your usual portion of some things is much larger than you thought and of others smaller. Then estimate, with a spot check every so often to stop the estimates drifting. This captures most of the benefit at a fraction of the cost.
3. Use package fractions. Half a package, a quarter of a package. Less precise than weighing, much better than guessing, and requires no equipment.
Option two is what we would actually suggest for most people. Estimates drift upward over time without recalibration, which is a well-documented pattern rather than a personal failing.
Volume versus weight
A specific trap: measuring by volume when the food's density varies.
A cup of one cereal and a cup of another can differ substantially in weight, and therefore in everything else. The same applies to chopped vegetables, grated cheese and anything that packs differently depending on how it was cut or how firmly it was pressed in.
Where a label gives both a volume and a weight, the weight is the reliable one.
Raw versus cooked

Decide whether you weigh raw or cooked, and always use a matching database entry. Mixing them produces errors comparable in size to a serving-size mistake.
What this changes about tracking

Consistency over precision. Trends over days.
If you estimate the same portion the same way every time, your totals are comparable to each other even when they are not exactly right. That comparability is what lets you see whether something changed.
