"Keto macros" sounds like a specification. It is a convention — three percentages that low-carb writing settled on, repeated often enough that people assume a regulator wrote them down somewhere. Nobody did. There is no US federal definition of a ketogenic macro ratio, and the numbers you see quoted differ between sources because they are traditions rather than standards.
That does not make them useless. It changes what you should expect from them: a rough shape for a day, not a target to be hit to the decimal.

What a macro ratio actually is
Three macronutrients supply the calories in food, and each supplies a different number of calories per gram. FDA's own labelling regulations put the working figures on the record: manufacturers may calculate calories using specific food factors, or the general factors of 4 calories per gram of protein, 4 per gram of total carbohydrate and 9 per gram of fat.
A macro ratio is just an allocation of a day's calories across those three, expressed as percentages. Written as fat/protein/carbohydrate, "70/25/5" means 70 percent of the day's calories from fat, 25 percent from protein and 5 percent from carbohydrate.
That is the whole idea. Everything difficult about it comes from the conversion.
The conversion, done twice
The formula is:
grams = (daily calories × percentage) ÷ calories per gram
A 2,000 calorie day at 70/25/5:

Notice what happened between the two tables. The carbohydrate percentage stayed identical at 5 percent, and the gram figure fell from 25 to 20 — because 5 percent of a smaller number is a smaller number. If you eat to grams, the percentage is a consequence. If you eat to percentages, the grams move whenever your intake does.
The trap: percentages are not stable targets
This is the single most common source of confusion, and it is worth spelling out with numbers rather than words.
Suppose a 70 kg person holds protein at 84 grams a day — the bottom of the 1.2 g/kg figure often cited for people in training. That is 336 calories of protein, every day, unchanged.
- On a 2,400 calorie day, 336 calories is 14 percent of intake.
- On a 1,600 calorie day, the same 336 calories is 21 percent of intake.
Same food. Same protein. Two different "macro ratios", seven percentage points apart, and nothing about the person's eating changed. A tracker showing percentages will report this as a swing, and people routinely try to fix it by eating differently when the arithmetic was never broken.
The practical rule that falls out of this: set carbohydrate and protein in grams, and let fat and the percentages land where they land. Grams are stable; percentages are derived.
Why fat is usually the remainder
The fat number gets the most attention and deserves the least. In practice, most people set a carbohydrate limit and a protein figure first, and fat fills the space that is left — which means the fat percentage is an output of the other two decisions rather than an independent one.
That is also why fat percentages look so high. Fat carries 9 calories per gram against 4 for the others, so once the carbohydrate and protein allocations are small in calorie terms, fat necessarily takes the large majority of what remains. A 70 percent fat figure is arithmetic, not an instruction to pour oil on things.
For orientation, FDA's Daily Value for total fat is 78 grams and for saturated fat 20 grams, both against a 2,000 calorie reference diet. Those are labelling reference figures rather than personal recommendations, and a high-fat pattern will exceed them. Where these ratios sit relative to the published reference intakes
Being direct about this is more useful than tiptoeing around it.
The National Academies set Acceptable Macronutrient Distribution Ranges for adults of 45 to 65 percent of calories from carbohydrate, 20 to 35 percent from fat and 10 to 35 percent from protein. They also set a Recommended Dietary Allowance for carbohydrate of 130 grams per day, derived from the average minimum amount of glucose used by the brain.
A ketogenic ratio is outside both of those on purpose. Twenty grams of carbohydrate is well under the 130 gram RDA and about 7 percent of FDA's 275 gram Daily Value for total carbohydrate. Seventy percent of calories from fat is roughly double the top of the acceptable range.
None of that means the pattern is unsafe, and none of it means it is safe. It means you are deliberately outside the population reference ranges, which is the specific situation where "check with a clinician" stops being boilerplate and starts being the actual advice. That goes double for anyone taking medication for diabetes or blood pressure.
You will also see 20 to 50 grams of carbohydrate a day quoted widely as the range associated with ketosis. Treat it as a repeated convention rather than a clinical threshold: no US federal nutrition authority publishes a carbohydrate cutoff for ketosis, and individual responses differ. Total carbs or net carbs — pick one and stay there
A macro target means nothing until you say which carbohydrate figure it is measured against.
A US Nutrition Facts label declares Total Carbohydrate with Dietary Fiber, Total Sugars and Added Sugars indented beneath, and sugar alcohol optionally. "Net carbs" is not on that list — it is a subtraction that you or your app performs afterwards, and different conventions subtract different things. A 20 gram limit on total carbohydrate and a 20 gram limit on net carbs are two genuinely different limits.

The other numbers people track alongside
Low-carb eaters commonly log more than three macronutrients. Two categories come up repeatedly and deserve accurate framing.
Electrolytes. Sodium, potassium and magnesium are frequently tracked in low-carb practice. FDA publishes Daily Values for them — 2,300 mg for sodium, 4,700 mg for potassium and 420 mg for magnesium against a 2,000 calorie reference — but whether your intake of any of them needs attention, and in which direction, is a clinical question and not one an app or an article can answer. Ketone readings. People measure these with blood meters, urine strips or breath devices.
