What ketones are
MedlinePlus puts it plainly: ketones are acids your body makes when it breaks down fat for energy.
Normally the cells in your body run on blood glucose, most of which comes from carbohydrate in food. When cells cannot get enough glucose, the body breaks down fat instead, and that process produces ketones. If fat is broken down too fast, ketones can build up in blood and urine, and high ketone levels make blood too acidic — a serious condition called ketoacidosis.
Two details in that description do real work later, so it is worth pausing on them.
The body makes three types of ketone. MedlinePlus lists beta-hydroxybutyric acid and acetoacetate among the other names for what these tests measure. Different test methods target different molecules, which is why the results are not interchangeable.
Producing ketones and having ketoacidosis are different states. Ketones appear whenever fat is being broken down for fuel. Ketoacidosis is what happens when they accumulate to the point of acidifying blood. Confusing the two in either direction is the origin of most bad information on this topic.
Ketosis and ketoacidosis are not the same thing

Diabetic ketoacidosis (DKA) is the most common form. MedlinePlus describes it as a medical emergency that often develops quickly and can lead to coma and be life-threatening. You will find "optimal ketosis" ranges quoted confidently across the low-carb internet. Those figures do not come from a US federal health authority, and this article is not going to repeat them as though they did.
