How the index is built

A test portion of a food containing a set amount of available carbohydrate is eaten, blood glucose is measured over the following hours, and the response is compared with a reference food.

The number is a ranking, not a property of the food in the way that its calorie content is. It describes how a group of people responded to a standardised carbohydrate dose from that food.

Why glycemic load was introduced

The problem with using a fixed carbohydrate amount is that it does not correspond to how people eat.

Consider a food that produces a strong glucose response per gram of carbohydrate but contains very little carbohydrate per serving. Its index is high; the practical effect of eating a normal portion may be small. The index reports the first fact and hides the second.

Glycemic load fixes this by scaling the index by the carbohydrate content of a realistic serving. It is a better answer to "what will this actually do", which is the question people are asking.

The limits that apply to both

They are averages from small groups. Testing involves a limited number of participants, and individual responses to the same food vary considerably. A published figure describes a group, not you.

Preparation changes the answer. Cooking method and degree, processing, and ripeness all affect the glucose response. A single value describes one preparation of the food.

Meals are not single foods. What else is on the plate — fat, protein, fibre — affects the response. Almost nobody eats a carbohydrate food in isolation, which is exactly how it was tested.

Neither says anything about nutritional quality. The index does not know about calories, fat, micronutrients or fibre. A food can produce a modest glucose response and still be a poor choice for other reasons.

Why neither appears on a label

Nutrition Facts labels carry total carbohydrate, dietary fibre, total sugars and added sugars — the FDA's label guide sets out what is required and why.

Glycemic index and load are not there. That is a reasonable signal about how much weight regulators place on values whose measurement conditions differ so much from ordinary eating.

It also means that if you want to use these figures, you are relying on external databases rather than on the package — and those databases carry the caveats above.

Where this sits relative to net carbs

Both are attempts to answer "which carbohydrates matter", and both are approximations.

Glycemic index and load measure a physiological response rather than applying a rule to a label.
